| PARENT OF A NEW RU STUDENT?
While you’re packing the sheets and towels, care packages and
extra phone cards, remember to be sure that your son or daughter also brings
the following with him or her when arriving on campus:
Information needed for the University Health Services
1. A copy of your health insurance card and your pharmacy
card (if you have one).
2. A history of son or daughter’s allergies to medications.
3. Complete immunization record (and make a copy for
yourself to bring to your first RUHS visit).
a. Rutgers
University REQUIRES that all students have immunity from measles, mumps
and rubella - be sure the RU form (mailed withthe admissions packet) has
been completed and sent to the university so that your son/daughter may
access his/her course grades later in the semester.
b. RUHS
recommends that in addition.
i. tetanus immunization is up-to-date (received at least in the past 10
years),
ii. that students have Hepatitis B immunity (your pediatrician should have
records),
iii. that varicella vaccine be received if your son/daughter never had
chickenpox, and
iv. that families consider Menomune (a meningitis immunization), particularly
for the first-year students (and especially those who live in residence
halls and/or smoke).

4. Name and phone number of son’s/daughter’s primary
doctor, and eye doctor and dentist.
5. If any chronic medical problems:
a. Name
of diagnosed condition,
b. Name
and phone number of any health care provider involved in his/her health
care,
c. Name
and phone number of any applicable psychiatrist or therapist.
6. If on medication: exact dose and mg strength and
number of times/day to take medication.
Information You May Want To Know
1. RUHS is accredited by the Joint Commission for
the Accreditation of Health Care Organizations, and have strict confidentiality
policies. We CANNOT release any information about your son/daughter without
his/her consent. Students must call their parents themselves, or authorize
us to call in the event of illness.
2. Our physicians, nurse practitioners, nurses, psychiatrists,
psychologists, counselors, health educators and other health professionals
are licensed, credentialed and certified in their specialties. See our
brochures for more information about our services.

has much information to help your son/daughter, including:
a. Records release form (to print out, have family physician
fill out, and send back to us),
b. Immunization form (if not yet completed, can be printed
out, completed and sent in),
c. Online brochures,
e. A description of all of our departments and health
centers,
f. Ways to get involved, from jobs and volunteering,
to taking courses and requesting programs. |
